POSITION SUMMARY :

Registered Nurse (RN)

The RN oversees participant health and wellness at Happy Life Adult Day Health Center. Responsibilities include conducting health assessments, administering medications and treatments, monitoring chronic conditions, coordinating care with physicians and families, supervising LPNs and nursing aides, and ensuring compliance with all healthcare regulations and care plans to promote participant safety and well-being.

Licensed Practical Nurse (LPN)

The LPN provides hands-on nursing care and supports the RN in maintaining participants’ health and comfort. Duties include medication administration, vital sign monitoring, assisting with daily health needs, documenting care, reporting health changes to the RN, and promoting a safe, supportive environment for all participants at Happy Life Adult Day Health Center.

Both will work under the general and direct supervision of the Program Director.

RESPONSIBILITIES:

  1. Provide or supervise the nursing care of each program participant.

  2. Participate in the quality assurance program for client care.

  3. Participate in the development and review of the participants care plans.

  4. Participate in the distribution, administration and storage of all medications

  5. Advise the program director of medical and related problems.

  6. Maintains proper attendance and punctuality to ensure that the facility is operated in an efficient and cost effective manner.

  7. Encourage and coordinate the keeping of scheduled appointments and having prescriptions filled promptly.

  8. Note any significant changes in the participant’s behavior and condition and report them to the Administrator.

  9. Ensure the completion and accuracy of the participants’ medical records as well as proving for their secure storage and confidentiality.

  10. Assist as necessary in the delivery of other center services.

  11. Attend and direct Inter Disciplinary Team meetings.

  12. Attend staff meetings, as scheduled.

  13. Perform the initial nursing assessment for each member, and write clinical progress notes as needed. Vitals must be taken the first five (5) days of attendance.

  14. Arrange for doctor’s appointments at the request of members. Provide daily list of appointments to Transportation to ensure proper transport.

  15. Administer prescribed medications, as necessary, and in accordance with the Happy Life Adult Day Health Policy and Procedure Manual (Ensure medicines and Class II medications are properly secured). Maintain existing relationship with local pharmacy in procuring prescriptions.

  16. Administer and read mantoux tests to new personnel.

  17. Posses complete familiarity with the local 9-1-1 emergency procedure.

  18. Notify Program Director of all emergencies and hospitalizations immediately. Notify emergency contact source, as appropriate, of concerns, suggestions or emergencies.

QUALIFICATIONS

  1. Registered Nurse, licensed by the Massachusetts Board of Registration in Nursing to practice in the Commonwealth of Massachusetts.

  2. Must have a verification card of current instruction in CPR.

  3. Must possess a basic proficiency in English, spelling, and math.

  4. Knowledge of basic computer programs, Word and Excel.

  5. Must have at least two years’ recent experience in the direct care of adults or chronically disabled persons.

  6. It is preferable that the experience has involved working with the aging and adults with chronic impairments and that all or part of the experience has been in a community health setting.

GENERAL STAFFING REQUIREMENTS:

  1. Free of communicable disease

  2. Able to physically/mentally perform tasks, as verified by a health screening that includes TB clearance

  3. Criminal record clearance

  4. Ability to read, write and understand English

  5. General understanding of the needs of the elderly and disabled population

  6. Willingness to learn
